How Can I Request Government Economic Assistance for Unemployment in California?
To request government economic assistance, you must submit a series of documents with which you will prove the need you have to receive the assistance, such as:
- Information about your last employer, including their name, the name of the supervisor (only if you had one), work address, postal code and the phone number of the place where you worked.
- You must also have a written statement indicating the date of your last day of work and the reason why you are no longer doing so.
- Similarly, you must bring a proof of your gross income from the last week you worked, starting with Sunday and ending on the last day you performed your duties.
- You must attach a work history for the last 18 months including all employer names as they appear on your paycheck, address (of the workplace and postal code), the dates you worked, how many hours you worked per week, how much you received per hour and the reason why you are no longer working.
